3.Counter-Notification

- Your signature; - Identification of the removed material and its previous location on the service; - A statement under penalty of perjury (or equivalent declaration) that you have a good-faith belief the removal was a mistake or misidentification; - Your name, address, and phone number; - A statement of consent to the jurisdiction of the courts located in Uttar Pradesh, India.
